Taskforce Pilot Project - One Stop Shop ( Permits)

PLEASE CONFIRM THAT YOUR EMPLOYEE[S] MEETS ANY OF THE FOLLOWING CRITERIA:
- has been tenured in employment with your business consecutively for the past five [5] (proof of such
required) or
- has been residing in Sint Maarten over the past five [5] with or without receiving either residence or labor
permits, and having an established employee/employer relationship

Document Requirements
- Labor Registration (valid)
- Application form completed ( MODEL 1A & MODEL 1B)
- Copy of diplomas & certificates of the non-national ( if applicable)
- Copy of Medical Insurance for the employee ( Non-national)
- Labor Agreement , signed by both parties ( if applicable)
- Copies of Payslips or receipts of salary payment to the non-national
- Declaration from the Inspectorate of Taxes , proof of income FLS. 36,000.- or more per year .Proof of
Income , Bank Statements , Bank Letter.
- Payroll taxes receipts ( Wage taxes, AVBZ , TOT , Profit Tax, SZV premiums) of current month for
the Employer ( business)
- Color Copy of Passport ( All pages) of the employee ( non-national)
** Including expired passports , where and if applicable **
- Color Copy of Passport of the Guarantor ( employer/ Business owner) ( bio page only)
- Civil Registry form ( not older than 3 months) of the employee ( non-national) ** if not registered ,
a declaration from the Civil Registry that the non-national is not registered **
- Two ( 2) Passport picture of the Non-national ( not older than 6 months )
MODEL 1E
- Payment receipt for the processing fee ( TWV)
- Proof of Payment – IND Fees ( including Waarborgsom)
- Authorization form for 3rd parties submitting on behalf of the Employer ( Applicant)
- Copy of previous Employment permits & Residence Permit ( if applicable)
- Declaration of Conduct ( Police Report)
** Not older than 6 months **
- Exemption letter from Minister of Justice ( approval to remain on island)
- Birth certificate with apostille ** translated by a certified translator in English or Dutch ** ( not older than 3 years)
- Marriage Certificate with apostille ** translated by a certified translator in English or Dutch ** (not older than 3 years)
- Tax Declaration , Income tax filings for the employee ( non-national)- if applicable
- Proof of Address ( lease agreement , Utility Bills)
- Proof of 5 years’ residence on St. Maarten ( with or without receiving either permits)
- Affidavit signed by Employer and Employee
